- There is a beta of midi-qol for dnd5e v4.1+. It is NOT ready for game play.
It can be installed from:

It requires dae 12.0.2+ which must be installed via manifest link:

Any issues with damage calculation/application are midi-qols's and not dnd5e's. Raise the issue on the midi discord rather than with the dnd5e team. - 11.5.0 requires dnd5e v3.3+
- 11.4.25+ are compatible with foundry v12. There are certain to be bugs so only use v12 for testing not actual game play.
- 11.4.14 is the first non-beta release of midi-qol for dnd5e 3.0. There are likely to still be issues, but it has been used for real sessions. It requires dnd5e 3.0 to activate
- To revert to the dnd5e 2.4.1 compatible version uninstall midi-qol and reinstall it. You must make a bakeup of your 2.4.1. world before moving to dnd5e 3.0+ to be able to go back to dnd5e 2.4.1
Midi-qol provides combat automation with almost all features being optional.
- Auto roll attacks and damage rolls
- Auto roll saving throws and/or prompt players/GMs to roll saving throws.
- Automatically check hits
- Automatically apply damage to targets taking into account damage immunities
- Automatically apply active effects to targets
- Automatically apply/remove concentration.
- Many "special duration" settings to expire effects
- Many flags to alter roll behaviour
